He was predeceased by: his parents, Dale Foulk and Ruth Foulk (Young); and his wife Carole Elaine Brant. He is survived by: his children, Heidi Boot (Mark) and Jeremy Foulk; and his sister Carol Jean Foulk Walton (Gerald) of Meadville. He is also survived by three grandchildren; two great grandchildren.
