He was predeceased by: his parents, Steven Gazdak and Katherine Gans Gazdak; his wife Patricia Torok Gazdak; his son Jeffrey S. Gazdak; and his brother Robert Gazdak.
He is survived by: his sons, Thomas Gazdak (Gina) of Jamestown, N. Y., Richard Gazdak (Denise) of Russell and James Gazdak of Russell; his grandchildren, Ashley Grubbs (Alex), Angela Gustafson (Michael), Katie Gathers (Eric), Heather Douvlos (Ryan), Jillian Gazdak (Kurtis), Sierra, Hannah, Alexis, Kelsey, Peyton, Devon and Dylan Gazdak; his great grandchildren, Olivia, Clara Gustafson, Nolan Grubbs, Remi Gathers and Marley Douvlos; and his friends, Bob and Alice Stofko. He is also survived by several nieces and nephews.
The family suggest memorial donations be made in Eugene's name to Hospice of Warren County, One Main Ave, Warren, Pa., 16365.
