He was predeceased by: his mother Patricia Bosh Kuzma; and his brother Bobby (Debra). He is survived by: his wife Melinda; his children, Phillip, Brandon, John, Jeffery, Krysta and Taylor; his father Robert Kuzma; and his siblings, Sandra, Kimberly (Steve), Michael and Nicole (Tim).
