She was predeceased by: her parents, Albert G. Warner and Emma Meyers; and also Richard Warner. She is survived by: her children, Michael Formica (Mary Beth), Mark Formica (Roberta), Lisa Formica (Lloyd) and Steven Formica (Linda); and her grandchildren, Erik, Emily, Megan, Nicole, Robert, Alison, Jacqueline, Sydney and Arden. She is also survived by her husband and children.
