She is survived by: her husband Nicholas Spak; her daughters, Mary Cathryn Bailey (Kenneth) of DuBois, Nickie Ann Smith (Joseph) of Davenport, Irene Rita Mennetti (Robert) of Punxsutawney, Jane Ann Petrick (Michael) of Punxsutawney and Ann Marie Burkett (Kevin) of Reynoldsville; her grandchildren, Melanie Kiehlmeier (Eric) of DuBois, Carrie Bailey of DuBois, Diane Holzman (Adam) of Oviedo, Doug Smith of Coraopolis, Bradley Smith of Davenport, Elizabeth Mohney (Doug) of DuBois, James Petrick of Punxsutawney, Nicholas Smelko (Sheena) of Delancey, Curtis Smelko (Josie) of Punxsutawney, Steven Smelko (Jessica) of Shippenville, Jordan Burkett, Josh Skaggs of DuBois and Jaelene Burkett (Emily) of Girard, OH; her great grandchildren, Samantha, Isabella Holzman, Logan, Caleigh Smelko, Arden and Mason Mohney; and her siblings, Leo J.
