He was predeceased by: his parents, Minnie Fordyce (Becker) and John Fordyce; and his siblings, Raymond Fordyce, John Fordyce, Joseph Fordyce, Bernie Fordyce, Elizabeth Miller, Dorothy Raab, Hazel Gudikunst and Jane Bright.

He is survived by: his wife Dolores C. Fordyce (Urban); his daughter Leslie F. Phillips (Nicholas) of Sinking Spring; his granddaughter Chelsea J. Phillips (Christopher Badinger); and his great grandchild Riley.

In lieu of flowers, donations may be made in honor of Jacob "Jack" H. Fordyce to St. Joseph's Roman Catholic Church, 1018 N. 8th Street, Reading, PA 19604 for the Roof Repairs.
