He was predeceased by: his parents, Jack W. Evans, Sr. and Blanche Evans (Burns); his son Jack W. Evans, III (Marcia E.); and his sisters, Barbara Hershey, Martha Reganhart and Susan Brown.

He is survived by: his wife Christine S. Evans (Querns); his sons, Jeffrey Scott Rogers of Camp Hill and David M. Rogers (Dawn Rogers) of Blandon; and his grandchildren, Sean Evans (Kristen Evans), Alex Evans (Nicole Evans), Jeffrey Rogers (Abby Rogers), Kayla Evans (Michal Menet), Kimberly Rogers (Dorian McKay), Jacob Rogers, Mikah Rogers and Easter. He is also survived by his 4 great grandchildren.

Floral tributes would be appreciated, or contributions may be made to Shriners Hospitals for Children in Philadelphia.
