She was predeceased by: her brother Barry Fontana; and her nephew Devin Fontana. She is survived by: her husband Michael G. Polyak III; her daughters, Rebecca Chambers (Jeffrey) of Wyomissing and Katherine Smythe (Robert) of Springfield; her grandson Benjamin Smythe; and her siblings, Paul Fontana (Marylee), Rebecca Snyder (Jeffrey), David Fontana and Susan Shott. She is also survived by her nieces, nephews.
