He was predeceased by: his grandmother Nancy Hernandez. He is survived by: his parents, Sandra Y. Hernandez and Lecadio Velez of Reading; his daughter Cassandra R. Hernandez; his stepson Andrew Petry; and his sisters, Jennifer V. Velez (Sergio Hernandez) of Exeter Township, Melissa Y. Galindo of Reading and Kimberly P. Velez of Reading. He is also survived by several aunt, cousins, and friends.
