He was predeceased by: his parents, Michael Kostiak and Olga Kostiak (Gambal); and his brothers, Harry, Gene and Michael.
He is survived by: his wife Jessica Wall; his children, Christine Kostiak of Ransom and David Kostiak (Colleen) of Ransom; his grandchildren, Amanda Thomas (Kyle) of Fl, Abigail Graham of Fl, Kyle, Troy and Isabelle Kostiak of Ransom; his great grandchild Lance Thomas of Florida; his sister Marie Stefanelli; and his sister-in-law Gloria Kostiak. He is also survived by many nieces, nephews, and cousins.
In lieu of flowers memorial contributions maybe made to Countryside Community Church.
