He is survived by: his parents, Barbara A. Clark (Doman) and Brian D. Clark; his stepmother Maria P. Clark; his children, Sommer Clark, Gabriel Clark, Katelyn Garcia, Emily Clark, Forrest Clark, Jr. and Rachel Clark; his siblings, Brandy Voytko (Kevin), Stephanie McManus (Michael), Jennifer Aaron (Steven) and Eduardo Arevalo (Erin); and his nieces and nephews, Chase, Gavin, Peyton, Michael, Faith, Aaron and Maximus.
