He was predeceased by: his parents, Anna Smatlak (Payerchin) and John Smatlak; his wife Betty R. Smatlak (Nicholson); his daughter Carol; and his siblings, John Smatlak, Paul Smatlak, Mike Smatlak, Edward Smatlak, Mary Durica (Smatlak), Anne Cole (Smatlak), Margaret Sipka (Smatlak), Dorothy Saltsman (Smatlak) and Betty Smatlak.
He is survived by: his son Jim Smatlak (Brenda) of Punxsutawney; and his siblings, Millie Shultis of Torrington, CT, Ruth Walton (Mark) of Sherwood, OR and Rev. Ken Smatlak of British Columbia, Canada. He is also survived by many nieces and nephews.
