She was predeceased by: her parents, Isaac Weiner and Sophia Weiner (Wachter); her sisters, Lorraine Forman, Alice Skurnik and Dorothy Golin; and her husband Jerry Field.
She is survived by: her children, Amy R. Lewis of Auburn and Steven I. Field (Laura) of Orwigsburg; her grandchildren, Sadie (Brock), Lucia, Julianne, George (Sierra) and Henry; and her great grandchild Sienna. She is also survived by numerous nieces and nephews.
In lieu of flowers, the family request contributions in Phyllis's memory be sent to: The Field Family Scholastic Award at Pottsville High School, 1600 Elk Ave, Pottsville, PA 17901.
