He was predeceased by: his wife Linda L. Ondik (Ewing); his parents, George J. Ondik Sr. and Johanna A. Ondik (Ragnone); and his siblings, Susan Ondik, Joseph Ondik and George Ondik Jr..
He is survived by: his children, Heather Sarik and Tom Ondik Jr. (Sharon Foreman); and his grandchildren, John Tyson, Kyleigh Ondik, Tyler Ondik, Luke Ondik, Christopher Rankin Jr., Michaela Foreman, Anthony Foreman and Zachary Foreman.
