He was predeceased by: his parents, John Bolcavage and Mary Bolcavage (Okal); and his sisters, Helen Prusak and Maryann Belluch. He is survived by: his wife Patricia Bolcavage (Petrusky); his daughters, Kelly Bolcavage (Louis Putzeys), Kara (Jason Stone) and Kristin (Rob Allison); his grandchildren, Devin and Melia Stone; and his brother-in-law George Petrusky of Auburn, PA. He is also survived by many nieces & nephews.
