He is survived by: his siblings, Carol Wolfe of Fairmont, WV and Carlton "Buddy" Hammack of Morgantown, WV.; his wife Rosalie Hammack (Chulick); his sons, Richard and Eric Hammack of Masontown Wv; his sister Susie; his daughter Shari; his great grandchild Abigail; and also, Richard Appleton of Georgia, Laura Appleton-Dixon of Morgantown and Deanna Minor of Hagerstown. He is also survived by sister, a brother, 10 children and many grandchildren, great grand-children, nieces and nephews.
