He was predeceased by: his parents, Joseph J. Kachurak and Margaret C. Kachurak (Capitan) of Larksville; his wife Christine Yustat; and his brother Michael. He is survived by: his sons, Frank and Adam (Karen); his grandchildren, Augustin and Aleksandria; and his siblings, Paul, Margaret (Peggy) and James (Jim). He is also survived by numerous cousins, sisters-in-law, nieces, nephews, and friends.
In lieu of flowers, Tom's family suggests that memorial contributions may be made to All Saints Parish.
