She was predeceased by: her son James A. Ellsworth; her siblings, Joseph, Florence, Irene, Rose and Elizabeth; her granddaughter Amanda Ellsworth; her great grandchild Kyle Callahan; and her parent William Suprum.
She is survived by: her parent Florence Butsavage Suprum; her children, Linda Pagoda (Paul), Cathy Levy (Henry), Hope Ellsworth, Trout Lake, Sharon Callahan of Port Matilda, Bill Ellsworth (Amy), Mary Kopetchne of Shavertown, Holly Ratowski of Wilkes-Barre, Kim Bonner (Dave) and Fishing Creek; her granddaughter Gena Callahan; and also Kristen Werner. She is also survived by 19 grandchildren; 12 great-grandchildren; and several nieces and nephews.
