He was predeceased by: his parents, John Ludvik and Mary Ludvik (Harbac); his wife Dorothy E. Ludvik (Hutter); his son Paul A. Ludvik; and his siblings, Joseph, John Steve, Andrew, Michael Ludvik, Rose Ludvik and Helen Lamolinara.
He is survived by: his sons, Thomas M. of Greensburg, James W. (Nancy) of Derry and David A. Ludvik (Margie) of Mutual; and his granddaughters, Amy, Sara and Allie Ludvik. He is also survived by several nieces and nephews.
