He was predeceased by: his grandfather John Simonson; and his aunts and uncles, Jimmy, Jackie, Darren Simonson, Brett Fasulka, Peggy, Kim Simonson and Kelli Fasulka.

He is survived by: his parents, Annie Simonson (Tony Lukasavage) and Thomas Fasulka Jr.; his grandparents, Sharon, Girard Kalinay and Thomas Fasulka Sr.; his brothers, Chris Simonson and Brett Fasulka; and his uncles, Ricky, Brian, Joseph, Bobby, Donny and Tommy Simonson.
