He is survived by: his parents, George D. Smith Sr. and Brendella Miller Humphrey; his fiancée Ann M. Ashworth of Plainfield Township; his sons, Jordan T. Smith, Torey Smith, Jayden Smith and Malik Smith; and his brothers, George D. Smith and Corey Smith.
