He is survived by: his son Thomas J. Cook, Jr. (Dahiana Turbides); his brother Joseph Cook (Kim); his niece Kimberly Hickok (Wayne Rector); and his great-nieces and great-nephews, Kerry Hickok and Wayne Rector III. He is also survived by numerous nieces, nephews and friends.
