He was predeceased by: his parents, Joseph Urbanik and Mary Urbanik (Hoza); his wife Naomi; and his siblings, Mary Miley, Agnes O'Brien and Joe Urbanik.
He is survived by: his children, Scott (Sandra), Catherine (Jim) and John (Rhonda); and his sister Pauline Smith. He is also survived by many grandchildren; many great grandchildren.
