She was predeceased by: her parent Jean Carol Hayden of Pittsburgh. She is survived by: her daughter Kayla Mydlowski (Rob); her grandchildren, Kassidy, Kason, Kiley, Kesley and Bella; her parent Brackney; her sister Cindy Carr (Ed); her nieces and nephews, Rob, Amanda, Brianna, Zach, Mila, Lulu, Hunter, Elias, Kloe and Ayden; and also, Rich, Lorrie (Mike) and Shawn (Chris).

If desired, Tammy's family suggests contributions to American Cancer Society or to the college accounts established by her for her grandchildren.
