He was predeceased by: his parents, James A. Anzalone and Joyce Nielsen (Herman); his brother Gregory; and his grandparents, James, Grace Anzalone, Fred and Dorothy Herman.
He is survived by: his sister Dana Swoyer (Jeff); and his children, Raquel Frye (Brandon), Nicole Anzalone, Jessica Gordon (Joe) and James V. Anzalone. He is also survived by three grandchildren.
