It is with great sadness that we announce the death of Patrick Chmelko (Philadelphia, Pennsylvania), who passed away on August 3, 2019, at the age of 25, leaving to mourn family and friends. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
He was predeceased by: his grandfather Daniel. He is survived by: his parents, Michael and Marianne; his sister Katie; and his grandparents, Florence Chmelko, Edward and Joyce Erwin. He is also survived by uncles, aunts, cousins and friends.
In lieu of flowers family would prefer donations in his name to St. Anselm CYO at the above mentioned address.
