He is survived by: his wife Joyce Church; his children, Cassandra Church, Leo Patrick Church, Jr. (Charkell), Hakeem Church (Cassie) and Isaiah Church; his grandchildren, Kamora, Blair, Alyssa, Saniya, Makhi and Layla; his cousin Patricia Gorum; and his siblings, Jesse, Sheila (Michael), Josephine (Michael), Tammy (Sonny), Xavier (Ivalee), Mary (Terrence), Larry and Henry. He is also survived by numerous nieces, nephews, extended family and friends.
