He is survived by: his wife Janet S. Racko (Wojtkowski); his daughter Tami Hazen (Scott) of Ft. Worth, TX; his great-niece Dejanae Lowery; his grandchildren, Stephanie Hazen (Matthew), Hunter Hazen (Madeleane) and Fisher Hazen (Makayla); his great grandchildren, Lane, Sebastian, George and Oakley; and his sister Kathy Schaming. He is also survived by many nieces and nephews.
