He was predeceased by: his siblings, Michael Cook and Pamela J. Brown. He is survived by: his wife Staci Ann Berkeihiser; his children, Hailey L. Berkeihiser of Wyomissing and Derick Sebastian Ruffini of Los Angeles, CA; and his siblings, Kenneth J. Berkeihiser (Kim) of Douglassville, Staci and Glen Cook (Kim Beattie-Hart) of Chesterfield, VA.
