He was predeceased by: his parents, John Maderik and Olga Maderik (Puckert). He is survived by: his wife Hilda Maderik; his children, Lori O'Rourke (Chief), Rourke of Derry Township, PD and John P. Maderik; his grandchildren, Keeghan P. O'Rourke of Palmyra, Jessica Maderik, John Paul Maderik of Queens, NY and Patrick M. O'Rourke of Manor Township, PD; his great grandchildren, Reaghan and Remington O'Rourke; and his brother Michael Maderik (Janis).
