He is survived by: his daughters, Heather Van Horn (Bill) and Amity Knight; his grandchildren, Madison Jolie Van Horn and Chase Van Horn; and his siblings-in-law, Jeffrey Lewis (Patti) and Mary Lewis Jesiolowski. He is also survived by Aunts, cousins, nieces, nephews, and great nieces and nephews.
