He was predeceased by: his parents, John D. Polachko and Ellen Polachko (Cromie); his wife Florence M. Polachko (Brocail); and his siblings, Ann, Helen, George, Pete Mike and Joe.
He is survived by: his children, Kim McCully (Robert) of Ramey, PA and John A. Polachko of Osceola Mills, PA; his grandchildren, Katie, Ashley (Erinn McCully), Angela and Mandy (John Polachko); and his great grandchildren, Abigail, Emily, Jacob, Maeleigh and Ava.
