He is survived by: his wife Nancy J. Kephart; his children, Leisa K. Myers (John) of Osceola Mills, PA, Luann Perna (Daniel) of Houtzdale, PA, Leslie D. Conklin of Pleasant Gap, PA and Donald W. Conklin, Jr. of Osceola Mills, PA; his brothers, Roy Conklin (Cathy) of Philipsburg, PA and Clarence "Boots" Conklin of West Decatur, PA; his grandchildren, Meggan Schoening, Ashley Boulton, Jared Myers and Andrew Boulton; and his great grandchildren, Kendall, Lainey, Alivia, Jessica and Emily.
