He was predeceased by: his parents, Blair Phelps and Cora Dinnison Phelps of Liberty; his daughter Brenda Blackwell; his grandson Nicholas Blackwell; and his siblings, Harlan, Laverne, Carol and Jean.
He is survived by: his wife Jane Teed of Oregon Hill; his children, Jeff, Bonnie of Oregon Hill, Lillian and Ed Smith of Wellsboro; and his son-in-law Kurt Blackwell of Oregon Hill.
