Though Jack may no longer walk among us, his spirit lives on in the memories held dear by his family and friends, a guiding light that illuminates their paths in times of darkness.
He was predeceased by: his father Jacob Baculik (Jack). He is survived by: his wife Susan Baculik; his mother Viola Baculik; his brother Tom Baculik (Melissa); his nephew Benjamin Baculik; and his godchildren, Maria Prazenica, Benjamin Baculik, Alex and Max Chobody. He is also survived by many aunts, uncles, and cousins.
The family will graciously accept flowers, but for those who wish to make an alternative contribution, donations to the Hillman Cancer Center are encouraged.
