He was predeceased by: his parents, George J. Costulas, Sr. and Catherine H. Costulas (Teti); and his siblings, Mary Christine Costulas, Diane Costulas and George J. "JuJu" Costulas.

He is survived by: his wife Georgette L. LaVanture; his children, Joseph J. Costulas of Renovo, Christopher (Williamsport) and Sarah Peslis of Lewisberry; and his grandchildren, Ava Renae, George Joseph, Athanasia Diamanto and Capria Marie.
