He was predeceased by: his parents, Irish and Philadelphia; his wife Kathy; his grandson Kyle; and his siblings, Frank and Mary.
He is survived by: his children, John (Denise), Jennifer LaPergola (Patrick) and Kristin Macrina (Joseph); and his grandchildren, Patrick, Taylor, Stephen, Amanda, Heather (George) and Sean. He is also survived by nieces and nephews.
In lieu of flowers, the family asks that donations be made in Jack's name to the Philadelphia Ronald McDonald House in memory of his grandson Kyle, continuing his legacy of giving and caring for others.
