He was predeceased by: his parents, Melvin George Sullivan and Grace Corine Werner Sullivan; his wife Nancy R. Sullivan; his sister Anna May Nevius; his half-brother Bob Kellenberger; and his brother-in-law Carlton Caldwell.

He is survived by: his sister Joyce Elaine Caldwell.
