She was predeceased by: her parents, Andrew Polak and Jane Ziemianski Polak; and her sister Joan Thomas. She is survived by: her siblings, Joseph H. Polak (Agnes) and Helene L. Reifschneider. She is also survived by several nieces and nephews, and great-nieces and great-nephews.
