He was predeceased by: his parents, Robert Lewis and Martha Lewis. He is survived by: his wife Shelly Lewis (Fray); his granddaughter Salem Jo; his daughter Samantha Jo Snelbaker (Jericho); and his siblings, Mike (Diane Lewis), Bobby (Nikki Lewis), Kimberly Lewis-Stahley and Stacy Simon. He is also survived by numerous nieces, nephews, great-nieces, and great-nephews.
