Smith (Everitt) of New Bloomfield; his daughters, Rachel Freeman of Mt Holly Springs and Kate Freeman of New Bloomfield; his mother Donna L. Smith (Rice); and his siblings, Keith E. Smith (Alisa) and Laurie C. Maxwell (Dave) of New Bloomfield. He is also survived by numerous nieces, nephews, and extended family.
