Czuba of Nazareth, Pennsylvania, born in Allentown, Pennsylvania, who passed away on May 4, 2021, at the age of 58, leaving to mourn family and friends. He was predeceased by: his parents, Dmytro Czuba and Olga Czuba (Faryna). He is survived by: his brothers, John G. Czuba of Danielsville, PA and Michael Karpenko of Annville. He is also survived by nephew and 3 nieces.
