He was predeceased by: his parent Joseph Anthony. He is survived by: his parent Irene Gazarik (Cieslinski); his children, Joseph Theodore Gazarik of Allegheny Township and Rebecca Renee Gazarik of Fort Wayne; and his grandchildren, Simon, Sebastian, Lenka Marie, Lukas Peter Gazarik, Cieslinski and Rago.
