He was predeceased by: his grandfather Daniel Kozick; his great-grandparents, Beulah Howell and Irene Corby; his uncles, Marvin Corby and Mike Kozick; and his cousin David Kozick.

He is survived by: his parents, Daniel Kozick of Dallas and Debbie Kozick of Trucksville; his fiancé Seanna Oliver; his sons, Brantley Leo Kozick and Kacey Walker Kozick of Nanticoke; his siblings, Bill Kozick (Amy), Danielle Kozick (Brian Bower) of Wilkes-Barre, Brandy Butler (Keith) and Deann Kozick; his grandparents, Bill and Shirley Corby; his uncle Bill Corby; his aunts, Sherry Corby and Lori Corby; and also Sam Vanhorn. He is also survived by numerous nieces and nephews, cousins.
