She was predeceased by: her spouse Thomas D. Mizak; her parents, John Kolesar and Ann Kolesar (Ondo); and her brother Bernard. She is survived by: her children, Thomas Mizak (Julie) of Kiowa, CO and Denise O'Connor (Mike) of Ravenna, OH.; her siblings, Eleanor Swanson (Don), Mary Potochnik (Robert), Angie Cartwright, Richard Kolesar (Cindy) and John Kolesar; her grandchildren, Nicholette Mizak, Brian Mizak (Paige), Steven Mizak, Michael Mizak, Chad O'Connor, Sarah Mizak, Kelley Prilla (Chris) and Timmy Mizak (Kaitelyn); and her great grandchildren, Mya, Brayden, Ronnie, Grayson and Henry. She is also survived by nieces and nephews.
In lieu of flowers memorial contributions may be made to The Gardens at Liberty Park, 9975 Greentree Pkwy.
