Jack of Mountain Top; his siblings, Michael Shannon of Mountain Top, Daniel Jack (Ashley) of Ohio and Jennifer Jack of Mountain Top; his nieces, Lyllian, Madilyn and Jaylyn; and his nephew Atticus. He is also survived by aunts, uncles and cousins.
