He was predeceased by: his parents, Daniel Sullivan and Frances Gallagher Sullivan; and his sister Mary Beth McCoy. He is survived by: his wife Sharon Conklin Sullivan; his children, Shawn Rutkoski (Scott Rutkoski) of Corning, NY and Daniel Sullivan (Mary Sullivan) of Limerick; his grandchildren, Sydney, Samantha Rutkoski, Madison and Harper Sullivan; and his siblings, John Sullivan of Freeland and Margie Frame of Texas.

In lieu of flowers, memorial contributions may be made to the American Diabetes Associations.
