He was predeceased by: his father Otis Cook, Sr.; and his grandparent Phillips. He is survived by: his mother Dawn Steel; his adoptive mother Joyce E. Phillips; his siblings, Otis, Jr. of Lewistown, Greg of Lewistown, Patrick "Beanie", John of Altoona, Shamus of Mount Union, Julie of Mount Union, Jerret of Mount Union and Jollene of Mount Union; his grandmother Faye Steel; and his uncles, Rodney and Mark Steel.

In lieu of flowers, memorial contribution may be made directly to the funeral home to help the family defer expenses.
