He was predeceased by: his grandparents, George, Julia Smalich, John J. and Mary Gulisek, Sr.; his close friends, William 'Bill" "Budda" Biller, Jr., John Rogacki and Katherine 'Katy" Gulisek; his father-in-law Aaron L. Ledbetter, Jr.; and his pet Harley.
He is survived by: his mother Jennie Smalich Gulisek; his wife Jackie Ledbetter Gulisek; his sons, John Patrick Gulisek, Jr. (Ashley) of Warren and Dakota Gulisek (Tabitha) of Mount Pleasant; his grandchildren, Logan, Lettie and Ethan; his pet Rosco; his siblings, James Gulisek of Greensburg, Jerome Gulisek of Conroe, TX and Jacqueline Sikora of Latrobe; his mother-in-law Joanna Ledbetter of Mount Pleasant; his siblings-in-law, Drew Ledbetter (Jennifer) of Tarrs and Maria Knotts; and also, Aaron of Kingwood, W. V and Jill Ledbetter of Mount Pleasant. He is also survived by several nieces and nephews.
