Madlenk Zubovic, Jr.; his fiancée Marianne Riggar; his sister Kimberly Zubovic (Teresa Conn) of Belle Vernon; his uncle John Bazilewich (Paulette) of Chipawa Lake, OH; his aunt Linda Madlenk Malaspina of Bethel Park; his cousins, Frank Malaspina (Linda), Mary Youngblood (John) of Atlanta and Julie Dunn; his caregivers, Brandon Walker of Mt. Pleasant and Chad Hull of Mt. Pleasant; his pet Max; and also Emily Dunn of Medina, OH.

In lieu of flowers, donations may be made to one's favorite Animal Rescue in memory of Jason.
